Thank you for your interest in working with one of our clinicians. Our intake process for NEW CLIENTS ONLY is as follows:

Step 1: Fill out the corresponding appointment request form below.

Step 2: We will process your insurance verification within 24-48 business hours. Please be on the lookout for an email from us with your verification details. We’ll need you to review it carefully and reply with your approval or any questions.

Step 3: Once you approve your verification, you’ll be invited to our portal to complete the rest of your paperwork. During this process, you’ll need to add your credit card, insurance, and fill out other to do’s. Everything must be completed before you can schedule. You’ll also use this portal for future communication with your clinician and/or administrative staff, including billing inquiries or general questions.

  1. Mental Health Psychotherapy Only: Please note that we do not offer medication services at this time; our services are limited to mental health psychotherapy services.

  2. Answering Questions: If you encounter any questions you’re unable to answer, please type “N/A” in the response field to continue with the form.
